With her fingers positioned high on the
vulva, the patient pulls aside the tissue around her clitoris, revealing its
location. This patient had a skin graft taken from her lower abdomen because
there was not sufficient penile and scrotal skin available to create an
acceptable vaginal depth. We can see that considerable penile skin has been used
to drape the vulva externally; perhaps this skin could have been better employed
to line the vagina.
In this view, with the patient’s fingers placed
lower and the labia more widely spread, we can see the size and position of the
urethral meatus. Based on the results I have seen, this example appears to be
fairly representative of Dr. Sanguan’s technique as of mid-2000.
